Since May of 2007, 71 residents have entered and received treatment at Second Spring.

The following charts and graphs are designed to overview the outcomes of our residents’ Recovery Plans.

Occupancy Rates are based on the average number of residents residing in the program per month, per year.  All years are based on a 12-month calendar year, with the exception of 2007, which is based on eight months due to the program inception date of May 2007.

Discharge Data:

Since the opening of Second Spring in May of 2007, 60 individuals have been discharged from the program and have transitioned into a variety of living situations in the community.  The types of living situations into which our residents have moved include: 

Independent Living: An apartment in the community with supportive mental health services.

Supervised Living:  A supported group living residence with supportive mental health services.  Examples of programs include: Hill House, Next Door, Safe Haven, Segue, Home Care, Kirby House, Single Steps.

Family Living:  A return to a family living situation.

A small percentage of individuals have returned to Vermont State Hospital (VSH) during their stay at Second Spring. Some have then returned to Second Spring and then moved on to the community.

There are 10 designated mental health agencies in the state of Vermont.  Second Spring collaborates with these agencies to develop and coordinate treatment, housing, and other services to ensure a successful transition back to the community. This chart reflects the percentage of clients served, upon discharge, by designated mental health agency.
